I regret to inform you that, effective September 20th, 2021, I am closing my practice. I will no longer be available to take care of your medical needs after that date.

Please arrange to have another physician take care of you. The American Society of Ketamine Providers provides a directory of other clinics on their website (https://askp.org/directory/). Once you have identified a physician or practice, please contact my office by September 20th, 2021 to obtain copies of your medical record.

For your convenience, I have attached a records release authorization (please include name, phone number, Fax Number, Email, and address of patient as well as new physician or practice on the form). Please request your medical records by September 20th, 2021.
Before September 20th, 2021, you may send your medical records request by email to info@ketamineintegrativemedicine.com , fax (Fax: 480-571-3061), or by mail to 633 E Ray Rd Suite 131 Gilbert, AZ 85296.

After September 20th, 2021, your records will be held by Dr. Annette Lusko D.O. for 6 years as required by A.R.S 12-2297. Future records requests can be made in writing to the following address:

About Dr. Lusko

Annette Lusko, DO, is a board-certified osteopathic physician who is committed to helping her patients lead pain-free, fulfilling lives. She opened Ketamine Integrative Medicine in Gilbert, Arizona, the only ketamine clinic in Arizona to offer both oral ketamine and ketamine infusion therapy to help patients overcome conditions ranging from depression and anxiety to chronic pain and migraines. 

Dr. Lusko earned her bachelor’s degree and Doctor of Osteopathic Medicine at Midwestern University in Glendale, Arizona. She served her internship at Community Westview Family Medicine in Indianapolis. During her family medicine residency at Mountain Vista Medical Center in Mesa, Arizona, she served as chief resident. Outside of her work at Ketamine Integrative Medicine, Dr. Lusko is also the chief medical officer of the largest addiction treatment program in Arizona.

At Ketamine Integrative Medicine, Dr. Lusko offers customized ketamine therapy to help patients optimize their wellness. She has seen first-hand how ketamine therapy has benefited patients from all walks of life with a wide range of health needs. She and her team provide personalized attention and care to fulfill their goal of breaking the chain of pain and suffering. 

The center is warm and comfortable, helping patients feel like they’re relaxing at home instead of receiving treatment in a medical office. While ketamine therapy isn’t currently covered by insurance, Ketamine Integrative Medicine provides affordable payment plans and super-bills that patients can submit for reimbursement.
